This is  a general non-descript term athletes seem to use to categorize all types of shin pain that aren’t stress fractures. There are a multitude of shin pains with a pathologically defined cause which have symptoms of pain along the inside or medial border of the shin, which worsens with exertion. However not all types of shin pains are “shin splints”. This undefined “pathological” term should be sort out under clinical diagnosis assessments to develop a more accurate and targeted treatment for a quicker and improved return to sport.

The origin of shin pain generally stems from:

1. Bone stress
2. Reduced blood flow (vascular insufficiency)
3. Inflammation
4. Increased pressure within the medial compartment of the leg due to inflammation, swelling, fibrosis of the fascia or thrombotic disease and vessel occlusions.
5. Squashing of the superficial peroneal nerve

6. Abnormal biomechanics

 

It is important to understand each reason has a clear differential diagnosis process. Once this has been identified each type of condition can then be treated for accordingly.
The most types of shin pain include; medial tibial periostitis, chronic exertional compartment syndrome, popliteal artery entrapment and muscle-tendon injuries/strains/tears/tendinopathies.

Some questions to ask yourself:

  • Did the pain suddenly come on?
  • Does the pain improve or get worse with warm ups or stretching?
  • Is there swelling?
  • Is there pins, needles, numbness or “electric” shooting type pains associated in the shin?
  • What have you tried to make it better?

Here are some treatments to try:

  • Massage of the calf and inside of leg
  • Icing the shin
  • Stretching the shin
  • Wearing supportive footwear
  • Avoiding hills when running/walking
If none of the above seems to rid the pain or reduce it, please come see a physiotherapist to assess your injury further and develop a treatment plan.
A physiotherapist can help with making the diagnosis by performing certain tests and assessments on the patient to reproduce their pain. They will then also look at the person’s running and walking biomechanics to determine any abnormalities. Once these have been determined a specific treatment plans may be developed to correct any issues and rehabilitate the area until the patient is pain free. Manual therapy, stretching, mobilizing and massage may be employed to correct key dysfunctional areas that influence the person’s movement. Additionally exercise plans may be prescribed to restore any muscle imbalances, inflexibilities, weaknesses and fatigue. Another factor in returning to sport is gaining appropriate control and proprioceptive balance between the core, pelvis and lower extremity.
A physiotherapist should be able to guide you through the initial acute/painful stage of the injury, modify your training routine and develop a rehabilitation program, then develop management strategies for injury prevention once returned to sport.

What are PRP injections and will they help my shoulder?

Tendon pain can be agonizing and debilitating. Seeking a solution in medication may not provide the relief, and you may be looking for an alternative treatment method.

Well, you have come to the right place. There is a technique that will help you say goodbye to pain faster and for longer. It is called the Platelet-Rich-Plasma ‘PRP’ injection treatment.

What are PRP injections?

PRP injections involve treatment that helps ligaments and tendons regenerate after accidents or injuries. It has also been seen to help reduce the amount of time used in rehabilitation and recovery. It may even in some instances get rid of the need for surgery for the injuries.

What does a PRP injection process look like?

  • A healthcare practitioner is going to draw a sample of your blood. The amount will vary depending on the area they intend to inject it.
  • They will then put it into a centrifuge that will separate the different components in the blood for about 15 minutes.
  • They will then prepare the separated plasma so that it is ready to be injected into the affected area.
  • An imaging tool will most likely be used to spot the area that needs the injection, and when they find it, they will inject it.

Can they work on tendons??

PRP injections have been seen to work. These offer good short-term improvement, less than 3 months, but do not confer a benefit in the longer term.

In one study, patients who had just had with rotator cuff operations were divided into two groups where one was a control group that did not receive treatment while the other one received PRP injection.

The two group were observed for were observed for 30 days. After the period, it was evident that the patients who received PRP injections had a lower pain score than those who did not have the treatment administered.

The conclusion of the study was that PRP reduced pain in the subjects in the first few months after an operation. The long-term results also showed positive results regarding cuff rotator healing.

Other studies have also looked at the combination of PRP and physiotherapy management. They show that at 26 weeks results seem to be promising for patients even in the late phase of patellar (knee cap tendon) tendinopathy. The addition of strength, endurance and power strength training for specific sports helps to improve the load capacity of the knee!

The results, therefore, allude to the fact that platelet-rich plasma injections are safe and useful for the treatment of teidnopathies.

Final thoughts

If you are having issues with tendon pains, it would be best to look at PRP injections as an alternative treatment not a first line, or a treatment in conjunction with load management. Do 12 weeks of strengthening  with a physiotherapist to achieve the best results by 26 weeks!

Claiming rebates from a private insurance provider or Medicare can be such a drag. Whether you do it online or have to visit their offices, the process is long, complicated and tiring. This is where HICAPS Go comes in.

What is HICAPS Go and what do they do?

HICAPS is a mnemonic for health industry claims and payment service. It is one of the leading claims and payment solution for healthcare providers in Australia.

HICAPS has been in operation for the last two decades, but the procedure was manual. They have now created the HICAPS Go App which allows you to make your claims and payment on the go.

How does it work?

Every time you get a bill from your service provider, they will require you to hand them your WorkSafe Victoria, health fund membership or Medicare card. They will then proceed to swipe it through a HICAPS machine that is electronic.

The HICAPS terminal will then process your claim automatically, and your health fund’s contribution will be included on the spot. All you will then need to pay for is the gap amount if there is any.

What is the procedure for signing up?

If you are an existing Medibank and AHM customer, all you need to do is download the app from the Apple app store or Google play store. But you need to have iOS 8.0 or newer for Apple users. Android users need iOS4.1 or newer.

Once you have the app, you will enter your private health membership information. Alternatively, you may use your debit card payment information.

Your account will then be set up, and you can begin making claims and payments. All you have to do is tap “approve” on your device whenever a practitioner creates a claim. 

 

Which services can you claim with HICAPS Go?

Sometime you may not be able to make claims for every doctor’s trip. What you can claim mostly depends on your insurance provider.

Are patient details private?

Every time you have to give someone your card, there is a likelihood they may have to go through its data. So how sure are you that your information is safe?

With HICAPS, all patients’ details are kept confidential. Only your health fund and your health care provider can see them. But even this parties need to use a secure encryption protocol. All these measures are put in place, so you don’t have to worry.

What is Physitrack and how can my physio get me access?

When you have been injured and need to undergo physiotherapy for healing, there is only so much your physiotherapist can do. You may need as many as 800 hours of therapy and exercise or more to recover fully. However, due to time and specialist constraints, that may not be possible.

Your physiotherapist may be very helpful but are you able to keep the pace when you get home? Most of the time, the answer to this question is no. Even though you are willing, the exercise you have “as assignments” may be very few and they get old very fast. We know that when the body gets used to any form of exercise, it is no longer challenged and this may likely cause the healing process to stall. This is where Physitrack comes in.

What is Physitrack?

This is a digital platform that allows physiotherapist, chiropractors, osteopaths and other professionals to assign exercises to their clients without spending an eternity creating programs.

It was founded in 2012 and apart from exercises; it also deals with education prescription, telehealth, and outcomes tracking and for physical rehabilitation and prevention.

How does your physio get you access?

First of all your physio needs to be a member of Physitrack. Once they have an account, they can either add you manually or through the integrated patient management system.

For them to complete your account, they will need your first name and date of birth. Your physio will then create an exercise program for you and assign you exercises. Once they have completed the process, you will receive an access code from Physitrack.

The access code will allow you to log in through your browser. Alternatively, you can also use the code to download PhysiApp, which is Physitrack’s app that is available on the web, Android, and iOS.

Your physio will then be able to monitor your progress without having to spend endless hours with you.

Why should you use Physitrack?

There are a couple of reason why this company is a cut above the other players in its industry and why you should choose it. Here are some.

  • It has an exercise library with over 3500 exercises. Additionally, they are all accompanied by videos and a commentary that is in both audio and written form.
  • They were the only fully exercise company that was chosen for the London Accelerator program. The program aims to identify and encourage digital solutions with the capability of improving public healthcare quality in the UK.
  • It gives informative feedback; there are features such as exercise adherence, pain levels and so on.

What is Physiapp?

Going to a general practitioner (GP), the optician or dentist is something everything does. We go for regular checkups even when nothing is wrong, or at least we don’t feel unwell. This is not the case with physiotherapists.

Whenever someone goes to a physio, it is either because they have a referral from another kind of doctor or when they have a terrible injury. What most people don’t know is that everyone can and should see a physiotherapist regularly.

To better understand why physios are just an important as GPs and other doctors, let us look at who they are and what they do.

Who is a physio?

A physio or physiotherapist is a medical professional who specializes in the science of movement that helps patients restore, maximize or maintain their strength physically. They also help deal with underlying physical issues, motion, overall wellbeing as well as function.

What does a physio do?

Physios have a broad scope of duties that they perform. They are in two broad categories, preventive and healing. There are different types are depending on their specialization. Let us look at some examples.
